We wanted to design a platform to share beacons of good practice and create a mosaic of individual moments of personalised care that will highlight that, despite the many pressures thrown at us, our health and social care professionals remain truly caring with the patient at the heart of all we do.

We invite you to share stories, highlighting actions that made the little things matter. We look forward to illuminating the wonderful care and compassion that you provide.

Whilst we want to share real stories from professionals and patients we also wish to ensure that the confidentiality of those involved is protected. Please be aware that we may have to edit your story to protect the confidentiality of those involved but we will discuss these edits with you prior to publishing your story on our website. Whilst we may remove names and organisations, it may be useful to include your role and setting, for example, senior nurse, acute hospital.

We want this website to generate debate and discussion but when posting on Twitter and Facebook please ensure that you follow the principles of confidentiality in ensuring that no patients or colleagues are identifiable; we cannot take any responsiblity for content posted on Twitter and Facebook.
